Eagle's Eye Workshop Surveys
(Psychometric Self-evaluation Questionnaires)

To help you prepare for your Eagle’s Eye experience, you are requested to complete and submit several psychometric self-evaluation questionnaires. A few of these simple questionnaires immediately need to be completed both "pre" and "post" workshop experience. Completing these questionnaires will help you to assess the current state of your mental health and feeling of wellbeing before the workshop experience. They will also allow you to assess whether any changes in your mental health are noticed after the workshop as a result of the experience. Participating in these questionnaires is optional but strongly recommended to help you assess and integrate your experience.

 

The two questionnaires for evaluation and comparison of your "pre" and "post" experiences are: the Beck Depression Inventory (BDI) and the Beck Anxiety Inventory (BDI).  After the workshop experience, you will also be asked to complete a Mystical Experience Questionnaire (MEQ) - to learn whether or not you had a mystical experience or just visual hallucinations. You will also be asked after your experience to complete an Expression of Spirituality Inventory (ESI) - to measure any perceived level of spirituality gained after your experience.

 

The information gathered from these questionnaires will not only be helpful to you in evaluating your experience. It will also be used by our practitioners, to evaluate the experience from their perspective, to consider modifications and other changes that may facilitate a more positive experience for our future participants and well as facilitating on-going research.  Again, these questionnaires are not mandatory, but completing them is strongly encouraged. Completing them all will be helpful to you in integrating your experience and, as well, in helping our practitioners in gathering information that will add to the scientific database on psychedelics. In effect, by completing these questionnaires, you are also helping us help others.  

 

Any information from any of these questionnaires that is used by our practitioners will be done so on a strictly anonymous basis to protect the privacy of our participants. (Please see our privacy and disclaimer statements for more information).

It is requested that you fill out the 2-part Beck surveys (below) both immediately before and immediately after your experience while any concerns or issues or questions regarding the workshop experience may be fresh in your mind. The Mystical Experience (MEQ) and the Expression of Spirituality Inventory (ESI) are to be filled out after the workshop experience.

INTERPRETING PART I (THE BECK DEPRESSION INVENTORY)

Now that you have completed part I of the questionnaire, add up the score for each of the twenty-one questions by counting the number to the left of each question you marked. The highest possible total for the whole test would be sixty-three. This would mean you circled number three on all twenty-one questions. Since the lowest possible score for each question is zero, the lowest possible score for the test would be zero. This would mean you circles zero on each question. You can evaluate your depression according to the Table below.
