The ESI questionnaire is used to measure any perceived level of spirituality gained from the workshop experience. It should be filled out and scored after your workshop experience. You may want to fill this form out at various intervals after your workshop experience as you may find that your answers may change as you integrate your workshop experience into your daily life.

This is a questionnaire which concerns your experiences, attitudes, beliefs and lifestyle practices pertaining to spirituality. Below are several statements. Read each statement carefully. Using the five point scale described below, rate the extent to which you agree with each statement as it applies to you and put your response in the space provided. There are no right or wrong answers. Please respond to every statement and respond as honestly as possible.

Now that you have completed the spirituality questionnaire, add up the score for each of the 32 questions (1 - 32) by counting the number to the left of each question you marked. The highest possible total for the whole test would be one hundred and sixty (160). This would mean you circled number five on all thiirty two questions. Since the lowest possible score for each question is zero, the lowest possible score for the test would be zero. This would mean you circles zero on each question. You can evaluate your spirituality experience based on your total score.
